Repeat script request

To request a repeat prescription, fill out the form below. All fields must be filled out. Make sure to provide the name of the pharmacy where you want to pick up your prescription, and your mobile phone number (or landline if you don't have a mobile). We may contact you if we need further information.

 

Most prescriptions will be sent directly to the pharmacy you have chosen - and you will receive a message to your mobile phone to confirm this. Otherwise, your paper script will be ready to collect from our reception counter 2-3 working days after your request is submitted (note: if ordering on a Friday or weekend or public holiday, allow 2-3 working days for your script to be ready).

 

The cost is $15.00, to be paid when you collect the script. For children aged 0-12 years there is no charge.

 

If you want us to send your script to a pharmacy outside Te Awamutu, please make sure to provide the full name and location of the pharmacy.

 

Payments can be made online to our bank account - Westpac 03 0442-0182067-001. Please put your name as the reference with your payment.

 

 

Ordering repeats for long term medications is easy if you have a log in for the Manage My Health patient portal. Ask at reception if you would like to sign up for Manage My Health.
